V3 Rating Sheets (1- and 2-Page Version)

1- and 2-page Rating Sheets are available to users at no cost. As discussed in the HCR-20 Version 3 Manual, these Rating Sheets are used to summarize the findings of evaluations.

The Ratings Sheets are available as PDF documents. You can download the PDF documents by clicking on the filenames below, and then print them as needed. You will need a PDF viewer to do this.

The Ratings Sheets are licensed under a Creative Commons Attribution-NonCommercial-NoDerivs 3.0 Unported License. Click on the license name to read the full legal details or a plain language summary.

1. You are free to use, copy, distribute, and transmit the Ratings Sheets (the “Work”);

2. Under the conditions that:

(a) Attribution — You must attribute the Work as being copyright © 2013 by the Mental Health, Law, and Policy Institute, Simon Fraser University and attribute authorship to Kevin S. Douglas, Stephen D. Hart, Christopher D.
